2010 Annual Conference
Registration Instructions
- Register for the conference using the form below.
- Reserve your hotel room(s). (This step is separate from conference registration.)
- Register in the Cincinatti Children's Hospital Medical Center CME system.
- Mail payment for conference fees to Injury Free.
Conference Fees
Full conference registration postmarked on or before Sept 14th:
- Member: $450
- Non-Member $500
Late Registration Fee: Postmarked after Sept 14th
- Member: $500
- Non-member: $550
Other information:
- Non-Member 1 day registration: $250 Must be postmarked by September 14th, 2010.
- Save $25 on all registrations postmarked prior to July 1, 2010
- ** Injury Free sites that have paid the $2,000 membership fee may submit registration forms for as many as four attendees free of charge.

Hotel and Airport Info
Hotel Reservations: You may register online or call the Intercontinental O'Hare Hotel at 1-800-341-7949 and ask for the Injury Free Conference rate of $104 per night. Our conference code is IFC. When reservations are made one night will be charged to your credit card. Reservations may be canceled without penalty up to 48 hours prior to the scheduled arrival and you will receive a full refund. After 48 hours the entire stay will be charged to your card with no possible refund.
Transportation: There is a free shuttle from the O'Hare Airport to the Intercontinental Hotel. Should you fly into Midway you will need to take a shuttle or bus to O'Hare and then arrange for the Intercontinental Hotel Shuttle.
Parking: If driving the hotel is located at 5300 River Road. We've received a discounted parking price of $11 if self parking. This does include in and out privileges.
Registration for CME Credits
Please note these important changes for this year's conference!
If you desire to receive CME credits for your participation in the conference, (1) when registering for the conference, you MUST check the "Request CME certificate" checkbox below, and (2) you also MUST create an electronic CME profile with this year's CME provider, Cincinnati Children's Hospital Medical Center. The e-mail address you use on the conference registration form must exactly match the e-mail address you use when creating your CME profile in CCHMC's online system.

Accreditation Statement
This activity has been planned and implemented in accordance with the Essential Areas and policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of Cincinnati Children's Hospital Medical Center and The Injury Free Coalition for Kids. Cincinnati Children's is accredited by the ACCME to provide continuing medical education for physicians.
This activity has been approved for AMA PRA Category 1 Credit™.
